Bring a valid government-issued photo ID, the ORI number and reason code from your screening instructions, your Social Security number, and any forms your employer or licensing board gave you.
The checklist
- A valid, unexpired government-issued photo ID, such as a driver's license, state ID or passport
- Your ORI number and reason code
- Your Social Security number. Missing details are a common reason a screening gets held up
- Any forms or instructions from your employer, school district or board
Before you go
- Do any step your employer asks for first. Some healthcare employers start your screening in AHCA's Clearinghouse before you're fingerprinted.
- Check that the name on your ID matches the name on your paperwork.
- Skip heavy hand lotion right before your visit.
For a group or mobile visit
Everyone being printed needs their own ID and their own ORI and reason code. Coworkers often share the same ORI if they're screened for the same employer.
